Consumer health data privacy policy
Effective
This policy applies to consumer health data, as defined by Washington’s My Health My Data Act and Nevada law, that Dr. Wendi LeBrett (“we,” “us,” or “our”) collects through wendilebrettmd.com and the emails we send from it. Because the site relates to a medical practice, information you give us may be consumer health data.
Categories we collect and how we use them
We collect:
- Waitlist information: your state, first name, last name, and email address.
- Information recorded when you join the waitlist or receive our emails: the date you joined, your IP address and the approximate location it indicates, whether you confirmed your email address, and whether you open our emails and click their links.
- The contents of any email you send us, which may include health information if you choose to include it.
We use it to:
- Run the waitlist, including confirming your email address, sending you information about the practice, and inviting you to enroll.
- Plan which states the practice serves.
- Set up your patient record if you become a patient.
- Respond to your messages.
Sources
We collect it directly from you, and automatically from your device when you join the waitlist or interact with our emails.
Sharing
We don’t share consumer health data with third parties or affiliates, and we don’t sell it. Service providers process it on our behalf under contracts that limit its use. We may disclose it if required by law.
Your rights
You have the right to:
- Confirm whether we collect, share, or sell your consumer health data, and access it, including a list of any third parties and affiliates we’ve shared or sold it to.
- Withdraw consent to our collection and sharing of it.
- Have it deleted.
- Ask us to correct it.
To exercise these rights, email us at hello@wendilebrettmd.com from the email address you used with us. We may ask for information to verify your request. If we decline your request, you can appeal by replying to our response.
Tracking across websites
We don’t allow third parties to collect consumer health data about your activities across websites or online services through the site.
Changes
If we make material changes to this policy, we’ll post the updated version here and change the effective date.
